چکیده

Vertebroplasty is often used to treat osteoporotic vertebral compression fractures (OVCF). The number of affected vertebrae can be reduced to a certain extent by some reduction methods including hyperextension and balloon dilation. There have been only a few studies on the effectiveness of vertebroplasty combined with hyperextension or balloon dilation. We collected data of 216 OVCF patients who underwent vertebroplasty between January 2010 and September 2014. These patients were divided into the balloon dilation group (66 patients) and hyperextension group (150 patients). Balloon dilation or hyperextension was performed under C-arm fluoroscopic guidance. Vertebroplasty was then carried out by injecting bone cement. After surgery, the degree of reduction, bone cement distribution, presence of bone cement leakage, anterior vertebral body height, Cobb angle, visual analog scale (VAS) score, and Oswestry Disability Index (ODI) score were measured. The results showed that both balloon dilation and hyperextension can relieve pain and improve the quality of life, and that there were no significant differences between the two groups in VAS score or ODI score. However, the outcomes of vertebral height restoration and improvement of kyphosis were significantly better in the balloon dilation group compared with the hyperextension group. intervertebral or anterior extravertebral cement leakage occurred in 19 patients, 6 were in the balloon dilation group and 13 were in the hyperextension group. There were no complications such as spinal cord injury, infection, and pulmonary embolism. In conclusion, both hyperextension and balloon dilation are safe and effective when combined with vertebroplasty. Balloon dilation is more effective for restoring vertebral height and improving spinal kyphosis.

Percutaneous Vertebroplasty in Iranian Patients with Osteoporotic Vertebral Fractures

Background: Osteoporotic compression vertebral fractures are common clinical problems. In those with refractory fractures, percutaneous cement augmentation has been suggested. The aim of this study was to evaluate the functional outcome of percutaneous vertebroplasty in Iranian patients with refractory osteoporotic fractures. Methods: We retrospectively studied 37 osteoporotic fractures in 28 p...

Does Percutaneous Vertebroplasty or Balloon Kyphoplasty for Osteoporotic Vertebral Compression Fractures Increase the Incidence of New Vertebral Fractures? A Meta-Analysis.

BACKGROUND Because of an aging population, osteoporotic vertebral fractures are becoming more frequent.Conservative therapy was considered the gold standard for treating osteoporotic vertebral compression fractures (OVCFs) in the past. Percutaneous vertebroplasty (PVP) or balloon kyphoplasty (BKP) as minimally invasive techniques are new treatments that are widely used for painful OVCFs. However...

Evaluation of the quality of life after vertebroplasty to treat compressive osteoporotic fractures☆☆☆

OBJECTIVE with increasing life expectancy around the world, fractures due to osteoporosis have become more common and the expenditure for treating them has also increased. The aim here was to evaluate the improvement in pain and quality of life among patients with compressive osteoporotic vertebral fractures undergoing vertebroplasty. METHODS eighteen patients with 27 fractured vertebrae unde...

Does Balloon Kyphoplasty Deliver More Cement Safely into Osteoporotic Vertebrae with Compression Fractures Compared with Vertebroplasty? A Study in Vertebral Analogues

Study Design A biomechanical and radiographic study using vertebral analogues. Objectives Kyphoplasty and vertebroplasty are widely used techniques to alleviate pain in fractures secondary to osteoporosis. However, cement leakage toward vital structures like the spinal cord can be a major source of morbidity and even mortality. We define safe cement injection as the volume of the cement injecte...

The effectiveness and safety of vertebroplasty for osteoporotic vertebral compression fractures. A double blind, prospective, randomized, controlled study.

BACKGROUND Vertebral compression fractures (VCFs) constitute a major health care problem in western countries. Several treatments modalities are available to relieve pain and allow increased activities. Percutaneous vertebroplasty (the injection of bone cement into the fractured vertebral body) is a relatively new procedure to manage patients with these fractures. The aim of this study is to ev...
